What is care management?

Care management is the process of assessing a client's current situation, providing recommendations or a plan of care based on the assessment and working with the family or legally responsible person to put the recommendations in place. Care Management is a private service and can be tailored to meet the needs of the client and the family or legally responsible person.

Who are care managers?

A Care Manager is a highly qualified individual who has advanced degrees in nursing, social work, gerontology, or a related field. Care Managers are trained to assess, plan, coordinate, monitor, and provide services for the client and their families.

What is the difference between a caregiver and a care manager?

An in-home caregiver will prepare meals, do light housekeeping and keep the person company. A care manager is a professional (nurse, social worker, gerontologist) trained to manage a range of health, functional, emotional, psychological, legal and financial issues related to aging. Often, clients hire care managers to recruit, train and supervise in-home caregivers to ensure medication compliance, nutrition, safety, and overall health.

How much do services cost, and who pays?

ACT Eldercare Services are billed on an hourly basis to our clients. Fee schedules are available on request. Generally, our services are not covered by Medicare or private insurance and are therefore private pay. Many long-term care insurance policies cover care management services, refer to your policy to see if care management is a covered benefit.

How much will we pay each month?

Monthly fees depend on the scope of the care plan. When we deliver the written care plan, we provide an estimate of the monthly charges involved to deliver the recommended services based on a set of assumptions.

Will we receive a report each month?

Yes. The care manager will provide a monthly status report as well as communicate any significant changes throughout the month. In addition, we provide a detailed invoice each month outlining the service we delivered, the date of the service, time elapsed, and the fee.

Is there any long term commitment required?

No. Our services are tailored to meet the needs of the client. The degree of involvement can be increased or decreased to meet the needs of the client and can be terminated at any time.
