Do I Need Help?
Signs that it is time to consult a Care Manager:
- Forgets things more often
- Has problems doing familiar things
- Puts things in strange places
- Forgets common words or uses the wrong words
- Has frequent problems with complicated tasks
- Has a major change in personality (confused, suspicious, or afraid)
- Is confused about where they are (or what time or day it is)
- Has lost interest in doing things
- Has sudden changes in mood or behavior
- Does things that don't seem to make sense
- Non-compliance with medications
- Failure to pay bills
- Unable to obtain help in case of emergency
- Leaving appliances on or unattended
- Frequent falls
- Poor personal hygiene
- Dressing inappropriately for the weather
- Poor eating habits, unable to obtain appropriate or adequate food
- Anxiety or depression
- Sudden deterioration in physical condition
- Lack of attention to appearance
- Recent weight loss
- Abuse or neglect is suspected
- Victim of financial exploitation
- Non-compliance with doctor's recommendations
- Recurrent hospitalizations
- Accumulation of mail, laundry or trash
- Vulnerability or open to suggestion
- Lost cash, jewelry or assets without recollection
- Difficulty in obtaining services for developmentally disabled children or adults or mentally ill adults
- New medical diagnosis or complex medical issues
- Family members live at a distance and need assistance with local resources and/or monitoring of their loved one
- Driving accidents
- Losing touch with friends
